We are excited to announced the opening of the Family Shade Mini-grant program! Please read the information attached and consider applying or sharing the information.

Focus of the Mini-grant program:

To encourage innovative and strategic community projects that will enhance the Title V National Performance Measures and support the implementation of the standards for systems of care for Children and Youth with Special Health Care Needs through measurable outcomes.

National Performance Measures:

  • Increase the percent of children with and without special health care needs who are adequately insured.
  • CSHCN receive the services necessary to make transitions to adult health care.
  • Children with and without special health care needs have a medical home.

Amount: $25,000

Rules and Procedures:

1.      Attendance at one of the 2 mini-grant information sessions is required.

2. The deadline for submitting the application for a mini-grant is February 28, 2022.

3. Finalists will present an oral presentation through Zoom the week of March 13, 2022.

4. Mini-grantee awardees announce on April 26, 2022.
