Join us as we discuss the concept of a Medical Home, what it means for your family, and ways to advocate.
In this hour long zoom training participants will:
Learn about what a medical home is and why it is used
Be able to identify the components of a successful medical home
Have an opportunity to ask questions about medical homes and advocacy when improvement is needed
